We will also collect information related to your health, such as information regarding the condition you are seeking help for. Information regarding on-going treatment and progress and your medical history.
We collect this information in a variety of ways. For example, personal information is collected through sign-up forms via email and during your consultation.

You book a consultation
When you contact via email or social media platforms
You interact with any other JR Pelvic Health Physiotherapy activities.
How Long Do We Keep Your Personal Information?
To ensure fair Processing, Personal Data will not be retained by JR Pelvic Health Physiotherapy for longer than necessary in relation to the purposes for which it was originally collected, or for which it was further Processed.
The length of time for which JR Pelvic Health Physiotherapy needs to retain Personal Data is set out in CSP and HCPC policies for record keeping. These take into account the legal and contractual requirements, both minimum and maximum, that influence the retention periods. All Personal Data should be deleted or destroyed as soon as possible where it has been confirmed that there is no longer a need to retain it.

Monitor, develop and improve our services and/or your experience (this may include personalising the advertising content you see).
Do We Share Your Personal Information?
Your information will not be shared outside of JR Pelvic Health Physiotherapy.
We do not share, sell, or rent your information to third parties, nor will we share your information with third parties for marking purposes.
Your consent will be obtained prior to sharing your information, GPs, other health care professionals, solicitors/legal teams, medical insurance companies.
How Do We Protect Your Personal Information?
We take the security of your personal information seriously. We have controls in place to ensure, as far as we can, that your personal information is not lost, accidentally destroyed, misused or disclosed, and is not accessed by anyone who is not authorised to do so.
We encrypt your personal information and use two-step authentication techniques wherever possible. We also use secure, platforms to manage your personal information.
Non sensitive information (your email address etc) is transmitted normally over the internet and this can never be guaranteed to be 100% secure. We cannot guarantee the security of any information you transmit to us, and you so do at your own risk.
Your Rights
Any personal information you supply will be treated in accordance with applicable data protection laws including GDPR and any other superseding legislation. We are committed to delivering the rights that individuals are entitled to under data protection laws.

The right to request a copy of the information we hold on you. To do this, please email jrpelvichealthphysio@outlook.com making it clear you are requesting a copy of your personal information and including the full details of what you require. You may be required to submit proof of your identity. We try to respond to all legitimate requests within one month, but, if your request is complex it could take us longer, we will keep you updated throughout the process. We may charge £10 for information requests to cover costs in providing you with details of your information we hold about you.
The right to correct any details about yourself that are incorrect, ensuring our data records are accurate.
The right to withdraw consent when we are processing your personal information on the basis that you have given us your consent to do so. You can withdraw consent at any time
